A novel hybrid architecture is proposed to tackle the challenges of diagnosing heart failure, a leading cause of global mortality. The Kolmogorov-Arnold Classical-Quantum Dual-Channel Neural Network (KACQ-DCNN) replaces traditional multilayer perceptrons with learnable univariate activation functions, leveraging quantum advantages. This 4-qubit, 1-layer model outperforms 37 benchmark models, including classical and quantum neural networks, achieving an accuracy of 92.03%. The KACQ-DCNN also surpasses other models in macro-average precision, recall, F1 scores, and ROC-AUC. Ablation studies highlight the synergistic effect of classical-quantum integration, improving performance by about 2% over MLP variants. Additionally, explainability techniques enhance feature interpretability, while conformal prediction provides robust uncertainty quantification.
